Medication Form (PA State Regulation 3270.133)
While attending our school there may be times that your child will need medicine.  We are happy to accommodate this for you, but in order to do so you must follow these guidelines:

• We require that parents complete and sign the Authorization for Medication form and the Medication Log. If the medication is a prescription, a signature from the prescriber is required on the Authorization form. Instructions must be very clearly noted on the form describing dosage and time to be given.
  
• We will not administer any medication to your child without the completed forms. This includes, but is not limited to, epi-pens, asthma medication, diaper ointment, sunscreen, chapstick or anything that would be considered a medication according to the PA State Licensing Department. Please ask the Director if you have any questions on medications.
  
• For medications that will be ongoing, your child’s form will need updated every 6 months. If a medication is discontinued, or dosage, time change, or medication change occur please notify the Director immediately and a new form will need to be filled out.
  
• If your child’s medicine can be given at home before or after school, we ask you to administer it at that time. We will only give medicine to children if it must be administered during the hours that the child attends school.
  
• Failure to comply with our Medication Policy will result in disenrollment from our school.
